Tier 1

4 VISITS PER YEAR

  • $50 per month

  • $570 for the year

Tier 2

6 VISITS PER YEAR

  • $75 per month

  • $810 for the year

Tier 3

8 VISITS PER YEAR

  • $100 per month

  • $1020 for the year

Additional visits over the number of contracted for the year are $150 per occurrence.

While membership fees cover access and extended services, traditional insurance may still be required for medications or labs.

Tier 4

12 VISITS PER YEAR

  • $150 per month

  • $1440 for the year
